–A general practitioner, who injures a patient when he attempts to treat an ailment that requires specialist care, even though there is a specialist available in rooms next to his surgery.

–A phlebotomist, who is not trained to take arterial blood, attempts to draw blood from a patient’s radial artery instead of a vein, with the result that the artery occludes and the patient loses the use of her hand.
